FAQ Fraudulent Activity

Q. Who do I contact for lost or stolen ATM, Visa® DEBIT, MasterCard or Visa Credit Card?
A. Call the Credit Union at (281) 856-5300 or 800-456-3228 and speak to a Call Center Representative during normal business hours. After hours, please call and select option "4" off of the main menu and leave the requested information and your card will be blocked.

Q. Fraudulent activity has occurred on my FCCU Credit/Debit card, what should I do?
A. If you find that there has been fraudulent activity on your FCCU Credit/Debit card, first, follow the steps in the previous Q&A to block your card. You will then need to fill out these 3 forms:

Credit Card Dispute Form
Plastic Card Interview Form
Plastic Card Transaction Affidavit

These forms will state that there has been fraudulent activity on your card and provide us further information to know which transactions you initiated and those that were fraudulent. Once these forms have been completed, bring them in to one of our branch locations so we can verify your identity. You can download the forms right here online, or you may obtain a form at any one of our 9 Houston area locations.

Q. I need to dispute a charge on my FCCU Credit/Debit card (duplicate charge, wrong amount charged, etc.), what should I do?
A. The cardholder needs to fax a written statement to FCCU at 281-856-0032 (Attention: Credit Card Department) explaining the details of the dispute. The written statement must include:

• Name of cardholder
• Date of transaction
• FCCU account number and card number
• Reason for dispute
• Amount charged on card
• Cardholder's signature
• Phone number where you can be reached

Q. What should I do if someone has forged my signature?
A. You will need to fill out the Affidavit of Forgery form stating that your signature has been forged. Bring the completed paperwork along with a police report to one of our branch locations so we can verify your identity. You can download the Affidavit of Forgery Form right here online, or you may obtain a form at any one of our 9 Houston area locations.
